The Money Cut: Wealth Buys Access, Not an Off-Ramp
Scene beat: Simon Cowell in 2023, admitting it out loud. The English TV personality and record executive behind The X Factor and Got Talent built a career on being the bluntest guy in the room, and eventually he turned that bluntness on himself. He acknowledged he'd had plastic surgery, and that he'd gone to extremes chasing a youthful look — extremes that left him looking unrecognizable, in his own words resembling a character from a horror movie. It hit his family, too. His young son Eric was reportedly quite distressed by the change in his father's face. So Cowell did the thing almost nobody on this list does: he reversed course. He had some of the filler removed, adopted a subtler approach focused on facials rather than extravagant alterations, and rebuilt the rest of the equation — balanced diet, serious hydration, and a stated commitment to not only looking good but feeling good.
Neven Ciganović is where the money reasoning goes sideways. He's a 45-year-old Croatian celebrity stylist and reality TV personality, known for his contributions to Croatian reality television and his extensive surgical history — and he recently disclosed a genuinely wild side effect from his most recent operation. He chose to have his third nasal operation at a clinic in Iran because of cost, and had to abruptly return to Belgrade. What followed was a persistent erection: priapism, caused by an allergic reaction to the anesthetic. It's rare, it requires immediate attention, and the French Association of Virology has emphasized that properly defined management is crucial to prevent complications and irreversible erectile dysfunction. Ciganović, to his credit or his detriment, remains committed to surgical enhancement, and expressed a desire for just one more procedure to reach his desired appearance and global recognition. Shot selection matters. So does venue selection.

Katie Price is the opposite of a cautionary tale with the lights off — she narrates her own surgeries like a director doing a commentary track. The English media personality and model maintained a natural look throughout her modeling career, which is the part people forget. It wasn't until she hit 30 that she decided to make changes. Now 45, she does unapologetically whatever she pleases. Her timeline: 1999, bust goes from 32B to 32DD with a refined nose shape. By 2006 she was well on her way to becoming totally plastic. By 2011 she proudly embraced it. 2016 brought fuller lips and a smoother, shinier face. Two years later her appearance at the National Television Awards was so strikingly different she was almost unrecognizable. From 2019 she's done annual breast augmentation, and each implant is estimated to weigh around 4.4 pounds. One doctor, Dr. Scott Felax, has expressed concern about the frequency and extent of her surgeries, emphasizing it could be a potential cause for worry.
Donatella Versace is the one you can read off a red carpet at a hundred yards. The Italian fashion designer and sister of Gianni Versace has a remarkably smooth, wrinkle-free face in her late 60s — a rare sight for a woman her age. The visible indicators suggest she hasn't skipped the chair: her nose has transformed to appear more slender and pointed, her upper eyelids seem less droopy, her cheeks appear fuller, and her lips have a different shape, possibly via cheek implants and lip augmentation. In 2014 she was photographed with an exceptionally shiny look that cosmetic surgery clinical director Mark Norfolk attributed to extensive Botox use. Norfolk's read is that Botox can create a stretched, shiny, line-free forehead, which aligns with Donatella's appearance — and that it's uncommon for a woman of her age to have no forehead lines at all, with eyes that appear somewhat half shut. We can't know the full extent of her procedures unless she says so herself, and it isn't accurate to claim she's had no work done. The middle ground is where the truth lives.

Michael Jackson is the entry that stops the room. His transformation is undeniably linked to plastic surgery, and comparing photos from his youth to the final images taken before his death in 2009 shows a profound alteration. Reports suggest he underwent a substantial number of procedures, some intended to correct previous surgeries that had gone awry. His journey began with the aspiration to refine his nose — and he was so apprehensive about his first rhinoplasty that he asked his sister LaToya to undergo the procedure first as a test. Once satisfied with the outcome, he proceeded in 1979, marking the inception of the journey. Dr. E.A. Gorbis, an expert in body dysmorphic disorder, speculates Jackson may have undergone as many as 100 surgeries, some altering his features to the extent that his ethnicity became difficult to discern. Dr. Wallace Goodstein, who worked alongside Michael's surgeons in the 1990s, revealed the roster included multiple nose jobs, cheek implants, eyelid surgery and the insertion of a cleft in his chin — and that Jackson would visit for multiple surgeries every two months, sometimes as many as a dozen procedures in a single visit. The exact number and nature remain uncertain. The pattern doesn't.
Jessica Alves, born Rodrigo Alves, is the statistical extreme. A Brazilian-British television personality born in São Paulo on July 30, 1983, to a Brazilian mother and a British father, she has undergone a staggering number of operations to alter her appearance. Her father's family had a history of European immigration to Brazil in the 1940s — initially agriculture, then supermarkets, shopping malls and real estate. She fondly recalls her grandfather buying her Barbie dolls during her childhood. Despite being born male, she always identified as feminine, expressing that by playing with dolls and crossdressing from around ages three to six, and she firmly believes gender should not be used to categorize individuals. During adolescence she experienced a hormone imbalance with excessive estrogen, which led to breast tissue development, and she faced bullying because of her unique characteristics. Her transformation has involved over 103 cosmetic operations costing just over £500,000, with 51 of them being plastic surgeries as of 2018 — and her nose procedures cost her her sense of smell.
